Forms for DTS
Thanks for your interest in doing a DTS with us!
Right now we are having technical issues with receiving registration fee payments, so we will ask you for that payment of ¥5,000 at a later date.
In order for the application process to be complete, we will need three character references. These references must be from:
1. Pastor
2. Employer/Teacher
3. Friend
You can send them a link to this online reference form.
If you will be under the age of 20 at the start of the school, a physical copy of the signature of parent or responsible party is required for the Release Liability Agreement and Consent for Treatment. Please print and have it signed before scanning and emailing to us. Alternatively, you can mail it to the address provided. The form is found here.
Once we receive these, we will evaluate your application and will communicate with you as soon as possible as to whether you have been accepted. Thank you again for your application. We are excited and blessed that you are interested in joining the work of Christ here in Tokyo!
